People I know who have been stationed in Africa have told me all about public health and sanitation- it is so far from how we manage things here that I don’t think we would have to worry about it. I realize there are outbreaks of communicable diseases in some areas with homeless/refugees in the US but that’s a lot different. But in Africa and other third world countries- sure, it could definitely happen.

I don't know about massive deaths from plague alone. It would take a lot of specific & world-wide conditions. 2020 seems too close to me, 3 years away.
But from a combination of things (nuclear, accidental or not, natural catastrophies, general spread of different diseases (virus, or things like diabetes & cancers), wars and terrorist attacks - all of that put together could do a lot of damage and faster than you might expect b/c exponential.
I remember reading the post with the "geyser effect". That stuck in my mind - energy building up below the surface and then BAM. Even if some things of that sort (like volcanos) give warnings, you never really know exactly when, or how bad the explosion will be.
I liked the domino effect post too. That's applicable to many things in life.
